Utilization of Dry Needling in Functional Neurologic Disorder

How Dry Needling Rewired a Paralyzed Limb: A Breakthrough FND Case Study

A 15-year-old girl arrived at our clinic with sudden paralysis. Four weeks later, she walked again. No surgery. No medication. Just neurologic dry needling.

Hi everyone, Natalie Hawkins here—physical therapist and instructor with Integrative Dry Needling (IDN). In this Clinical Relevance video, I break down how dry needling activated neuroplasticity in a Functional Neurologic Disorder (FND) patient—leading to a remarkable recovery.

Understanding FND: When the Brain “Short-Circuits” 🧠

Functional Neurologic Disorder (FND), previously labeled as conversion disorder, occurs when the brain’s software misfires, causing:

Motor dysfunction: Sudden paralysis, weakness, tremors and or seizures
Sensory loss: Numbness, altered sensation.
Triggers: Often linked to stress or trauma.

🧩 “In FND, the brain isn’t structurally damaged—it’s just misfiring.”

The Case: Teen Athlete with Sudden Paralysis 🩺

🔹 Patient: 15-year-old female athlete.
🔹 Symptoms: Left leg paralysis, sensory loss, severe low back/abdominal pain.
🔹 MRI Results: Normal brain & spine (ruling out stroke/injury).

🩻 Clinical Findings:
➡️ Strength: 1/5 in the left lower limb.
➡️ Reflexes: Hyperactive on the left side.
➡️ Sensation: Almost completely absent (except dorsum of the foot).

🚨 Despite normal imaging, she couldn’t move her leg. This was a central nervous system issue—NOT a structural injury.

The Treatment: Rewiring the Brain with Dry Needling 💉

Inspired by neuroplasticity research in post-stroke recovery, I designed a 3-step protocol to reactivate motor pathways.

Step 1: Paravertebral Needling (L2-L5) 🧬

🔹 Goal: Stimulate spinal circuits linked to motor/sensory function.
🔹 Technique: Two-inch needles inserted at key spinal levels that correspond with her motor/sensory
loss (L2-L5).

Step 2: Contralateral Homeostatic Points 🔄

🔹 Goal: Leverage cross-body neurology to reawaken dormant pathways.
🔹 Why? Stimulating the unaffected limb activates shared spinal and brain networks.

Step 3: Sensory & Motor Retraining 🎯

🔹 As sensation returned, I progressively needled the affected limb.
🔹 Combined with physical therapy & graded functional exercise for full motor recovery.

📅 Treatment Frequency: Needling 1x/week for 4 weeks + PT 2-3x/week.

The Results: From Paralysis to Functional Gait 🚶‍♀️

Week 1-2: Partial sensory return in the left leg.
Week 3-4: Strength improved to 3/5, walking independently.
Post-Treatment: Near-normal gait—watch the video to see her transformation.

💡 “Dry needling didn’t just reduce inflammation—it rebooted her nervous system and restored function.”

Why This Worked: The Neuroscience Behind the Recovery 🔬

🧠 Motor Cortex Activation:
Dry needling stimulates the brain’s motor control areas, reestablishing movement patterns.

🔥 Systemic Inflammation Control:
Reduces neurogenic inflammation, a key factor in FND flare-ups.

🔄 Cross-Body Neurology:
Contralateral needling jumpstarts spinal circuits by leveraging shared neural pathways.

Key Takeaways for Clinicians 🎯

Think Beyond Muscles: Use dry needling to reset central nervous system function.
Start Proximally: Paravertebral needling primes the spinal cord for motor recovery.
Track Sensory Changes: Sensory return often precedes motor gains in neurologic conditions.

Beyond FND: Broader Rehab Applications

The same principles apply to:

🏋️ ACL Rehab: Enhance quadriceps motor control post-surgery.
🧠 Stroke Recovery: Improve motor initiation & spasticity.
Chronic Pain: Reset maladaptive brain patterns driving persistent pain.

💡 “Dry needling isn’t just about pain relief—it’s about unlocking the nervous system’s full potential.”
