I have been recently referred a patient for Dry Needling for low back pain that is currently completing home dialysis 4x/week secondary to complications with previous AAA repair (2020) and history of CABG. Would she be appropriate for Dry Needling?

Below is what is listed within the manual as precautions, seems to be pretty broad? 

 

14. DN should not be used in the presence of vascular disease, including varicose veins.

Cody,

Dialysis is not a contraindication for DN for LBP- follow our 2:2 rule of course.  Vascular disease is general by design as each case is different. if an area is vascularly compromised in anyway the decision to create lesions has to be looked at closely to be sure they have the blood flow to heal.  Varicose is simple since we try to avoid needling any vasculature - needling does not help the aesthetics of varicosities so there is no indication to needle them.

Hi Cody! 

As Frank summarized well, Dialysis and AAA repair/CABG are not contraindicated. However, I would take into consideration the overall health of the patient when determining the correct dosage for treatment to optimize results and minimize soreness. 

Be sure to perform the QST to give you an idea of the status of there nervous system in addition to their health history to make a good decision regarding dosage. 

Other things to consider:

-needle length

-#of lesions/needle

-gauge

-what you do with the needle: basic insertions vs manipulations or ENS

 

Initially it's best to talk more, needle less.
