<p>I wondered if anyone had suggestions of smaller locked carts or cabinets that you like to use to store your dry needling supplies.  One of the offices I work at is ordering supplies and doesn't have a ton of space to house a huge cart.  It has to be locked, so I wondered if anyone had suggestions.</p>

                        <content:encoded><![CDATA[<p>I have a unique case that I am wanting to get input on from other clinicians: </p>
<p>I have a 70 year old female who had a knee replacement (hinged knee) in Jan of 2024.  She had PT at another clinic and reports good recovery post surgery.  In December 2024 she sustained an avulsion fracture to the inferior pole of the patella.  The ortho immobilized her in extension for 5 weeks and then she began PT to regain strength at another clinic.  She never lost the ability to fully extend the knee actively, however the patella on the involved side rests significantly higher than the uninvolved side. She also has a persistent quad lag with SLR that has not improved even as the MMT tested strength of the quads has shown improvement.   She came to me last fall with her primary complaint being the knee will buckle and cause her to fall where she will have to catch herself with an AD or furniture.  I have treated her for over 50 visits working on strengthening of the knee extensors, hips, static balance, dynamic balance and functional strength/balance (i.e. step ups unsupported).  Her strength and her level of function have improved however she still will have occasional knee buckling averaging 2x per week, placing her at a high risk of falling.  The MOI for the buckling varies, sometimes planted and twisting, sometimes standing in place, sometimes while walking.  </p>
<p>My question is has anyone treated something similar and can offer some additional ideas to incorporate?  Would there be any benefit from dry needling the surrounding homeostatics, and/or femoral n. distribution? </p>

<p>I have a patient with a splenectomy scar that runs from her navel to her xiphoid process; I wanted to make sure since it runs up so high that it was still safe to needle so long as I stay superficial with a 0.5in needle and angle the needle horizontally/under the scar not AP?  Based on my anatomy apps I am still clear of the lung field, but wanted to double check and see if anyone else has done any dry needling to a similar scar?  </p>

It's thought by pelvic health PT that she has pudental nerve entrapment. Do your courses teach how to treat this and are there things that I can focus more on that is taught in the 2 certification courses? L4-L5, inferior and superior cluneals. </p>
<p>She has pain down lateral leg to her knee occasionally. </p>

<p><span>I have a patient who came in for a consultation and is approximately three years out from cancer treatment but continues to experience persistent tremors. The onset appears to be related to specific cancer infusions. The patient has seen multiple neurologists, and treatment to date has focused primarily on medications, including seizure medications, with minimal improvement.</span></p>
<p><span>For additional context, the tremors appear to start in the feet and lower legs and then progressively work their way upward into the arms and hands.</span></p>
<p><span>Given that symptoms have persisted for three years without meaningful improvement, I am exploring whether dry needling could be a reasonable adjunct or trial option. Before attempting any intervention, I wanted to gather feedback from others who may have experience, research insight, or clinical considerations related to dry needling in this population.</span></p>
<p><span>Any thoughts on precautions, muscle selection, treatment rationale, or outcomes you have observed would be greatly appreciated.</span></p>]]></content:encoded>
